Student-Athlete Forms

*NOTE:  Reminder that we have the ARMS software system to help maintain online forms and NCAA compliance as well as assist in communication with you. All forms EXCEPT the physical exam form will be completed online in the new ARMS software system. Freshmen and incoming transfers, you will be notified about accessing your profile on that system on or before July 15th.  All student-athletes, the 2022-23 forms will be assigned to you by July 15th. They all must be completed before you are allowed to practice.

Please read the following page about what forms you can expect to complete online using workflow requirements. You must still schedule a time with a physician to get a physical exam as noted below and you will upload the signed form to your profile when it is available. 
 
Rockhurst Forms to be completed by all student-athletes each year (Returners and Newcomers). All forms are required to be completed before a student-athlete will be allowed to practice.
  • Pre-participation Physical Form (PDF): Download and print form to take to physician for your annual exam. Must be uploaded to your ARMS profile before you can practice, and administration staff will approve or deny the form based on the information about physical exam requirements found below on this page.
  • Student/Parent/Guardian Athletic Insurance Notification:  Requires online signatures of both student-athlete AND parent/guardian. This will also require you to upload a current copy (front and back) of your health insurance card.
  • Student-Athlete and Parent Information Form:  We will require you to update your contact information and campus address for the year and then update or verify your parent’s/guardian’s information on your ARMS profile before the start of each year.
  • Release of Liability: Requires either the signature of student-athlete if an adult, or both student-athlete and a parent/guardian if the student-athlete is not over the age of 18.
Rockhurst Forms to be completed by incoming freshmen and transfer. Returners will only need to update the form or validate no changes since the previous year.
  • Sickle Cell Trait Test Result Form: You must submit a previous result showing if you have or do not have the sickle cell trait. Requires either the signature of student-athlete if an adult, or both the student-athlete and a parent/guardian if the student-athlete is not over the age of 18. Student-athletes who entered Rockhurst before the fall of 2022 were allowed under NCAA rules to sign a waiver from providing the test result. Those student-athletes do not need to provide a test result unless you have a result and would like to submit it for the sports medicine staff to have on file.
  • Family Education Rights and Privacy Act (FERPA) Form: Provides us with guidance about to whom we can speak about your student records and about which student records we are allowed to speak to those individuals. It also gives us permission to provide letters of recommendation or phone references for you during graduate school, scholarship, or job applications.
  • Rockhurst Web and Social Media Release Form: Tells us if you approve to have information about you placed on our RockhurstHawks.com athletics website and extends to our athletics and team-based social media pages as well.
  • Student-Athlete Information Page:  Your opportunity to provide the Sports Information Department about your biographical and past sports accomplishments (letters earned, awards won, etc.) to use on the Rockhurst Athletics website or for other publicity.
NCAA and GLVC Forms to be completed annually by all student-athletes before you are allowed to practice.
  • NCAA Student-Athlete Statement Form: Approves our ability to communicate with stakeholders across campus and at the GLVC and NCAA to discuss areas of eligibility and affirming you have not damaged your amateurism status since the previous year, as well as a few other necessary items.
  • Drug Test Consent Form: Provides you with the penalties if you ever test positive during an NCAA mandated drug test. You must also agree to allow the NCAA to test for drugs to be allowed to participate in your sport(s). Requires either the signature of student-athlete if an adult, or both student-athlete and a parent/guardian if the student-athlete is not over the age of 18.
  • NCAA Injury Research Approval Form: Form asks if, for the purposes of research and improved treatments, you will allow documentation about any injuries you might suffer be shared with the NCAA to help future student-athletes. Your name will not be associated with the documentation, just your type of injury and result after treatment.
  • GLVC Web Release and Sportsmanship Agreement Form: Form for the Great Lakes Valley Conference office staff has two parts. The first provides permission to the GLVC to put information about you and use your picture on their website for any possible future story, award, or publicity the GLVC might want to do. The second part is the agreement that you will abide by the GLVC Sportsmanship Conduct Statement and understand the penalties that can be imposed by the conference for breaking that statement. You must sign the second part to be allowed to participate in competition in your sport(s).
Attention Deficit Hyperactivity Disorder (ADHD) Guidelines:

"Criteria for letter from prescribing Physician to provide proper documentation to the Athletics Department/Sports Information staff (PDF) regarding assessment of student-athletes taking prescribed stimulants for Attention Deficit Hyperactivity Disorder (ADHD), in support of an NCAA Medical Exception request for the use of a banned substance."

On behalf of the Rockhurst Athletics staff, best wishes to our new and returning student-athletes. Please review and act on the following items:

Physical Exam
You must be examined by an American Medical Association (AMA) certified physician. A physician (and M.D. or a D.O.) must sign the form.
 
Physicals performed by a physician’s assistant, nurse practitioner, chiropractor, or physical therapist will not be accepted.

Physicals performed in a doctor’s office by a physician’s assistant or nurse practitioner but under the supervision of the physician will be accepted only if the physician signs the form.
 
Please make sure the physician clearly writes their certification (M.D. or D.O.) on the form when they sign it.

Insurance – Athletics Participation
Parents and Student-Athletes: Please carefully read the Student/Parent/Guardian Athletic Insurance Notification Form with information about the athletics participation accident insurance carried by Rockhurst University through the NCAA and Mutual of Omaha. This is an “excess policy” that will only be used in the event of a bodily injury sustained during an authorized athletic event. This plan is in addition to your required primary health insurance coverage provided by you, your parents or as an optional purchase through Rockhurst.

Student-Athlete Move-in, First Practice Dates
Move-in for student-athletes participating in Soccer and Volleyball is Thursday, August 11, 2022. Move-in for student-athletes participating in Cross Country is Tuesday, August 16, 2022. Student-athletes participating on other sports team may move-in with the general student body.
 
Student-Athlete Team Meetings
The All Student-Athlete meeting will be held at a date To Be Announced. Each team will also meet at the beginning of the school year for orientation and compliance rules education. Your coaches will inform you when your specific sport team meeting will be held.

Expectations of Excellence:
  • This is a great time in your life, so take full advantage of the opportunities Rockhurst has to offer. You’ll remember this time the rest of your life.
  • Play hard and compete.
  • You represent yourself, your family, your team, your coach and Rockhurst; we will hold you accountable for your actions.
  • Prepare for your life after college and after sports - study, go to class (not an optional activity). Use college athletics and your time at Rockhurst to get your degree.
